A prospective cohort study has found that intramuscular botulinum neurotoxin injections significantly improved sleep quality in adults living with cervical dystonia, although the treatment did not substantially reduce symptoms of anxiety or depression.
Cervical dystonia, a neurological movement disorder characterised by involuntary neck muscle contractions, often involves nonmotor symptoms that add to patients’ daily burden. These may include sleep disturbances, mood disorders and reduced quality of life.
Evidence regarding the impact of botulinum neurotoxin injections on these symptoms has remained limited and mixed.
The study followed 29 adults with cervical dystonia, with a mean age of 61.6 years, who received care through a university hospital clinic and a specialised private physiotherapy practice.
Researchers assessed sleep quality using the Pittsburgh Sleep Quality Index (PSQI).
After treatment, the mean global PSQI score fell from 10.1 at baseline to 7.41, indicating improved sleep quality.
The mean difference of 2.65 was statistically significant, suggesting that sleep may respond positively to botulinum neurotoxin injections, at least in the short term. These findings highlight sleep disturbance as a potentially modifiable nonmotor symptom in cervical dystonia.
Researchers found only slight changes in anxiety and depression scores after treatment, with no statistically significant improvement.
Quality of life measures also showed little overall change, despite better sleep outcomes.
The authors suggested that persistent psychiatric symptoms may require additional psychological support alongside botulinum neurotoxin therapy.
They called for larger controlled studies, noting that while injections help motor symptoms, their impact on mood-related nonmotor symptoms appears limited, highlighting the need for multidisciplinary care.
