Liberal Blood Transfusions May Lower Repeat Heart Attack Risk, Study Finds

Recent research suggests a liberal blood transfusion strategy may reduce recurrent heart attack risk. It may benefit patients with anaemia after an acute myocardial infarction.

Investigators emphasised that additional studies are required before changing current clinical practice.

The analysis compared liberal and restrictive blood transfusion strategies in patients suffering from acute myocardial infarction with anaemia.

A liberal approach generally provides transfusions when haemoglobin levels fall below 10 g/dL, whereas a restrictive strategy typically waits until levels decline to 7-8 g/dL.

Researchers observed that patients receiving the restrictive strategy showed a slightly higher risk of the combined outcome of death or recurrent myocardial infarction. They also noted a higher rate of cardiac-related deaths in some analyses.

Nevertheless, the evidence remains uncertain because several findings were driven by individual studies and require further validation.

The findings suggest that maintaining higher haemoglobin levels through a liberal transfusion strategy could offer cardiovascular benefits for certain high-risk patients.

Researchers cautioned that the largest trial did not show a statistically significant difference in the primary outcome after 30 days.

Clinicians should continue making transfusion decisions based on individual patient conditions and existing medical guidelines.

Experts added that anaemia commonly complicates acute heart attacks and remains associated with poorer outcomes.

Identifying the safest transfusion threshold continues to be an important area of cardiovascular research.

The authors called for larger studies to confirm whether a liberal transfusion strategy should become standard treatment.
