The human body often sends subtle warnings long before symptoms appear, with mild blood-sugar imbalances frequently going unnoticed.
In India, nearly half of those with diabetes remain undiagnosed until complications have begun.
Experts assert that using modern biochemical markers such as Fasting Plasma Glucose (FPG), HbA1c, Oral Glucose Tolerance Test (OGTT), and others is critical to detect early abnormalities and prevent long-term complications.
Dr Ravi Sankar Erukulapati, Senior Endocrinologist at Apollo Sugar Clinic, Hyderabad, explained that the American Diabetes Association (ADA) has established cut-off points for accurate diagnosis: FPG ≥ 126 mg/dL indicates diabetes, while 100–125 mg/dL signals prediabetes.
A random plasma glucose level≥ 200 mg/dL with symptoms confirms diabetes.
OGTT, particularly useful for high-risk patients, defines diabetes at a 2-hour plasma glucose ≥ 200 mg/dL, while 140–199 mg/dL indicates prediabetes.
HbA1c, reflecting average glucose over three months, confirms diabetes at ≥ 6.5% and prediabetes at 5.7–6.4%.
However, alternative markers like fructosamine or glycated albumin may be needed in cases of anaemia or haemoglobin disorders.
Dr Mahesh DM, Consultant – Endocrinology at Aster CMI Hospital, Bengaluru, highlighted that these markers show how the body processes sugar before and after meals.
Insulin levels, C-peptide, and lipid profiles further reveal insulin resistance and metabolic health.
For symptomatic patients, one abnormal reading suffices for diagnosis, whereas asymptomatic individuals require two abnormal readings.
Experts stress regular screening for high-risk groups such as those with family history, obesity, PCOS, hypertension, or sedentary lifestyles.
Early detection allows personalised intervention, improving long-term health outcomes.
Research confirms that no single test is perfect. A 2023 meta-analysis in Frontiers in Medicine showed that combining HbA1c ≥ 6.5% with FPG ≥ 126 mg/dL increased sensitivity.
Relying solely on HbA1c risks missing nearly half of true diabetes cases, particularly in diverse populations.
Combining FPG with HbA1c or fructosamine improves early detection and reduces the need for OGTT, supporting large-scale screening efforts.
In India, late diagnosis of diabetes is common, often emerging after complications have set in.
Biochemical markers such as FPG, HbA1c, OGTT, fructosamine, and C-peptide provide a robust framework for early detection.
Understanding and using these tests intelligently transforms early diagnosis from a statistic into a lifesaving intervention.
