Sore Throat After Cancer Treatment: What It Could Mean

A sore throat may seem minor, but it can cause concern for people undergoing or recovering from cancer treatment.

While it may result from an infection, doctors say treatment-related effects and other conditions can also cause throat discomfort. It does not automatically indicate cancer recurrence.

Dr Hitesh Singhavi, Consultant Head & Neck Oncosurgeon at KIMS Hospitals, Thane, said chemotherapy and radiotherapy can cause mucositis, inflammation, reduced saliva and dryness, all of which may irritate the throat.

Other possible causes include viral or bacterial infections, fungal infections such as candidiasis, acid reflux and local irritants.

Treatment depends on the underlying cause. Doctors may recommend adequate hydration, saliva substitutes, mouth and throat care and pain relief for treatment-related symptoms.

Fungal infections may require antifungal medicines, while bacterial infections may need antibiotics.

A sore throat alone does not mean that cancer has returned. However, doctors may investigate recurrence or a second lesion if symptoms persist, worsen or occur alongside other concerning signs.

Patients receiving chemotherapy, radiotherapy or immunotherapy need particular care, especially when their white blood cell count is low.

Dr Raziuddin Khwaja, Consultant, Medical Oncology, Head and Neck Cancer Institute of India, said doctors should consider the patient’s symptoms, treatment, overall health and blood counts before deciding on medication.

Patients should seek prompt medical advice if a sore throat is accompanied by fever, chills, breathing or swallowing difficulties, severe weakness, persistent cough or rapidly worsening symptoms. Fever can be particularly concerning when immunity is weakened.

Difficulty eating or drinking, mouth ulcers, dehydration and a significant decline in overall health also require attention.

Doctors may examine the mouth and throat, conduct a flexible endoscopy and order blood tests. Depending on the findings, they may also recommend CT, MRI or PET-CT scans or a biopsy.

Experts advise cancer patients not to start antibiotics without medical guidance. A sore throat requires identifying the cause first, followed by appropriate treatment, rather than automatically assuming an infection or cancer recurrence.
